Reminder:
Dear Parents and Guardians,
Your child recently had the opportunity to participate in a mobile vision clinic – the Vision Van – at
Trimble Local Schools. The organizations that provided the clinic would like to know your opinion
about this service and whether it benefitted your child. You can help by completing this short survey:
www.bit.ly/TrimbleElem_JrHigh_VisionVan
This survey is administered by the University of Cincinnati Evaluation Services Center in partnership
with ResultsOHIO, which provided the clinic.
You can access the survey via your smartphone, tablet, or computer/laptop. The survey takes no
more than 15 minutes to complete. No identifiable information is asked in the survey and all
responses are anonymous and kept confidential.
Again, your response is greatly appreciated and valued as it will help improve the program so that
more children may benefit.
If you have any questions about this survey, please feel free to contact Sara Stacy, Research
Associate at the UC Evaluation Services Center at sara.stacy@uc.edu.
Sincerely,
Hilary Coffman, RN, BSN
Wellness Coordinator
